A strong and growing evidence base exists for the use of mindfulness-based interventions to prevent relapse in major depression and for the self-management of chronic physical health conditions (e.g. pain), but the evidence in other domains of mental health work is still emerging. Much work is being conducted outside the evidence base and standardised protocols, and by individuals with varied levels of experience and training. The (mis)perception of mindfulness as a ‘simple technique’ belies the complexity and skill needed to deliver a mindfulness training that has real therapeutic and transformative power. We propose a framework to help clinicians think through the suitability of mindfulness for their particular client group with the intention of providing guidance for thoughtful decision-making.

Sense of time in children with ADHD: Effects of duration, distraction, and stimulant medication

A recent theory of ADHD predicts a deficiency in sense of time in the disorder. Two studies were conducted to test this prediction, and to evaluate the effects of interval duration, distraction, and stimulant medication on the reproductions of temporal durations in children with ADHD. Study I: 12 ADHD children and 26 controls (ages 6–14 years) were tested using a time reproduction task in which subjects had to reproduce intervals of 12, 24, 36, 48, and 60 s. Four trials at each duration were presented with a distraction occurring on half of these trials. Control subjects were significantly more accurate than ADHD children at most durations and were unaffected by the distraction. ADHD children, in contrast, were significantly less accurate when distracted. Both groups became less accurate with increasing durations to be reproduced. Study II: Tested three doses of methylphenidate (MPH) and placebo on the time reproductions of the 12 ADHD children. ADHD children became less accurate with increasing durations and distraction was found to reduce accuracy at 36 s or less. No effects of MPH were evident. The results of these preliminary studies seem to support the prediction that sense of time is impaired in children with ADHD. The capacity to accurately reproduce time intervals in ADHD children does not seem to improve with administration of stimulant medication. (JINS, 1997, 3, 359–369.)

George Eastman may not have been a father, but to the residents of Rochester, New York, the principal founder of Eastman Kodak was a doting uncle. In addition to donating millions of dollars to Rochester-area schools and hospitals, Eastman reportedly paid to remove the tonsils of every child in town. Even after his death in 1932, his company looked after of its employees and retirees. For example, Kodak refused to use its size to negotiate for cheaper medical care for its Rochester employees. If it had, most small businesses in the area would have likely faced higher insurance rates and the prospect of eliminating health benefits for their employees entirely. Only about seven percent of Rochester residents do not have health insurance today, compared with fifteen percent nationally.
That may change, however. Faced with growing foreign competition and demands for higher returns, last summer Kodak threatened to desert its long-standing approach to buying insurance and encouraged its 44,000 retirees to join a health plan that excluded two high-priced hospitals.
